Who is eligible for CDPAP?

To qualify for CDPAP, a person must:

  • Be eligible for Medicaid
  • Need help with daily activities
  • Be able to direct your care
  • Choose your caregiver 

Who is eligible to be a caregiver for a CDPAP consumer?

Eligible caregivers are:

  • 18 or older
  • Family members
  • Friends
  • Parents

Who cannot be a caregiver in the CDPAP program?

You cannot qualify as a caregiver if you're under 18, the spouse of the patient, or a designated representative for the patient.

How do I enroll in the program?

You'll need to follow these steps to enroll in the program: 

  • Enroll in Medicaid Managed Care or Managed Medicaid Long-Term Care.
  • Choose a fiscal intermediary like Elite Choice.
  • Recruit a caregiver.
  • Request a medical order for home care from your physician.
  • Schedule an in-home assessment.
  • Wait for approval and authorization that will initiate services.

How can I become a paid caregiver?

After the approval of your application, you'll only need to complete a physical exam as well as complete payroll documents.

How do caregivers receive payment?
