Who is eligible to be a caregiver for a CDPAP consumer?

Who cannot be a caregiver in the CDPAP program?

How do I enroll in the program?
You'll need to follow these steps to enroll in the program:
- Enroll in Medicaid Managed Care or Managed Medicaid Long-Term Care.

- Request a medical order for home care from your physician.
- Schedule an in-home assessment.
- Wait for approval and authorization that will initiate services.
